Required Forms
**ALL ATTACHED FORMS ARE REQUIRED TO BE TURNED IN AND ANY ATHLETE DONATION YOU CHOSE TO MAKE NEEDS TO BE PAID BEFORE AN ATHLETE IS PERMITTED TO TRYOUT. COACH RUTTENBERG NEEDS AN ELECTRONIC COPY OF ALL YOUR FORMS. EMAIL YOUR COMPLETED DOCUMENTS IN ONE PDF TO: [email protected]
All LPHS Boys' Soccer players must turn in completed and signed copies of each of the below attached forms. Copies of each form may be obtained by clicking on the name of the form below. EACH AND EVERY ONE OF THESE DOCUMENTS IN PDF GROUP 1 & 2 IS REQUIRED. NO EXCEPTIONS.However, if you are not a transfer student, ignore PDF number 3.
NOTES:
*The IHSA Pre-participation Examination Form must be completed by a physician (doctor), and also must be be signed by the player AND the parent or legal guardian of the player. See form for details.
*The IHSA Concussion Information Sheet and the IHSA Performance Enhancing Substance Testing Policy Forms are required IN ADDITION TO the Pre-participation Examination Form. Parents and Athletes must acknowledge and sign.
*The CPS Player Record must also be signed by a Parent or Legal Guardian. Please understand that the note "I will not represent any other organizations in the same sport while a member of the team" does not prohibit a player from playing with a "Travel" soccer team before or after the LPHS Boys Soccer Season, but such participation during the LPHS soccer season is precluded by CPS and the IHSA.. *The Student Athlete Code of Conduct is REQUIRED FROM EVERY PLAYER. No player will be rostered to a Team until we have received the completed form. LPHS Soccer and the entire Coach Staff fully support the Student Athlete Code of Conduct, and all Coaches will enforce it to the letter.
*Finally, carefully read the HANDBOOK, I PROMISE and THE ROSTER ACCEPTANCE SPOT so you are familiar with the rules and regulations of the LP SOCCER Program.
