NO STUDENT CAN TRY OUT WITHOUT:
2. A completed and uploaded Player packet (see link at bottom of page) that includes a valid sports physical
Please follow the attached Guide to walk you through the process of ensuring compliance and eligibility for tryouts and playing a sport at LPHS. Athletes not registered or in compliance are not allowed to attend open gyms/conditioning, try out or compete on any team.
Important Note/Reminder: A sports physical is valid for 13 months and must be completed/signed and dated by a licensed medical professional. The physical must include authorization to compete in IHSA sports.
