Athletic Registration

Please complete the following steps to register your child for Lincoln Park Athletics.

1) Download and complete the player record packet 

 

2) Have a completed IHSA pre-participation physical.  Physicals are valid for 13 months.  This must be completed by a doctor.  Once completed, you will have to scan or take a picture of the document to upload.

 

3) Complete the registration form.  This is where you will upload your documents.  Documents must be in a pdf format.  

*Note* - The player record packet is a fillable form, however, you will have to scan the Athletic Physical in order to upoad.  The Athletic Physical must be completed by a doctor.
