Physical Examination Forms must be on file by the start date (first tryout).
 
A student-athlete interested in participating in a spring sport must complete a player record packet.
  1. Download and complete the player record packet.
  2. Have a completed IHSA preparticipation physical. Physicals are valid for 13 months. This must be completed by a doctor. Once completed, you will have to scan or take a picture of the document to upload.
  3. Complete the registration form. This is where you will upload your documents. Documents must be in PDF format.
*Note* - The player record packet is a fillable form, however, you will have to scan the Athletic Physical in order to upload. The athletic physical must be completed by a doctor.

Lincoln Park Athletics
Level 1 Volunteer - Lincoln Park Athletics is looking for as many parents as possible to become Level 1 volunteers.  This allows flexibility for our coaches and aides Lincoln Park in meeting certain CPS requirements.  Among other things, Level 1 Volunteer Status allows parents to act as chaperones and drive athletes to / from athletic events.  To become a Level 1 volunteer please use the provided link.  Please reach out to our Volunteer Coordinator, Ms. Vivian Troche ([email protected]), with any questions.
