Students must complete the following steps to take the survey:
1) Click on the student survey link: http://survey.5-essentials.org/cps/survey/student/
2) Enter your Student ID (8 digits) and Birth Date
3) Click 'Login'
4) Complete the survey
5) Click ‘Submit survey’
